- Step 14: Write the state where mediation and arbitration will take place.
- Step 15: Fill in the name of the state whose laws govern the agreement.
- Step 16: Add any additional terms you want to include in the contract.
- Step 17: Both parties will sign the contract in the spaces provided, and include their printed names and mailing addresses.
